Applications can be obtained by calling the Board
office and speaking to a staff member or by
e-mailing us. You will be asked the following
questions: |

Are you applying for a Physical Therapy or Physical
Therapist's Assistant license?

Are you applying via examination or endorsement?
You are applying via examination if you have
not taken and passed the National Examination. You
are applying via endorsement if you are licensed in
another jurisdiction and have passed the National
Examination.

Are you US or foreign-educated?

In
addition to the completed and notarized application,
the Nevada State Board of Physical Therapy Examiners
requires all applicants to submit a recent
photograph, fingerprints, references, original
transcripts, verifications of licenses, transfer of
examination score, etc. PTA's are also required to
submit original transcripts from High School.
PT Applicants - $300
PTA Applicants - $200
All Applicants - $51.25 for fingerprinting
Exam Applicants - $25 for an exam processing fee
Exam Applicants - $350 for the National Physical
Therapy Examination

The
process takes approximately 4-5 weeks. Those
applying via examination may require additional time
as the applicant must sit for the examination.

Applicants
are encouraged to contact the Board office for
licensure status.

Foreign Educated
Physical Therapists

In
addition to the requirements listed above, all
foreign-educated physical therapists are required to
obtain a credentials evaluation from the Foreign
Credentialing Commission on Physical Therapy (FCCPT).
The FCCPT is the only organization recognized by our
Board. This process takes approximately 4-6 months.
The credentials evaluation must reflect no
educational or credit deficiencies. We also require
passing TOEFL/TSE/TWE and proof of licensure from
the country of training.
