LICENSE VERIFICATION
The annual renewal process runs through July 31, 2008.
The on-line license verification information will be updated
2-3 times per week as needed. Phone calls to verify the
renewal of a license will not be accepted during this time.

information contained on this site is updated as
changes
occur and can be considered a primary source for
verifications.

Licensees requiring
verification to another licensing jurisdiction or
other entity should complete a “Request for License
Verification” form. The form can be located under
the “forms” tab.
The Nevada State Board of Physical Therapy Examiners
does not charge for verification. Verifications are
processed within 24 hours of receipt. |
